MEADOWRIDGE AFC
MEMBER REGISTRATION APPLICATION (2027)

TERMS AND CONDITIONS OF REGISTRATION

DISCLAIMER AND INDEMNITY

Participation in football activities organised by Meadowridge AFC (“the Club”) is entirely voluntary and undertaken at the member’s own risk.

The Club, including its Executive Committee, officials, coaches, volunteers, organisers, sponsors, and representatives, shall not be held liable for any injury, illness, death, loss, theft, or damage to personal property arising from or connected to participation in any training session, match, tournament, transport arrangement, participation-related travel, or other Club activity, whether held at home or away, except where such liability arises from gross negligence or wilful misconduct.

MEMBERSHIP AND DISCIPLINARY RIGHTS

Membership of Meadowridge AFC is granted subject to compliance with the Club’s constitution, rules, codes of conduct, and policies.

No person shall be regarded as a registered member of the Club until their membership application has been reviewed and approved by the Executive Committee.

The Club reserves the right to suspend, revoke, or terminate membership at any time for just cause, including but not limited to misconduct, breach of Club rules, non-payment of fees, behaviour detrimental to the Club, or failure to comply with disciplinary requirements.

Any such decision shall be made by the Executive Committee in accordance with the Club’s rules and procedures.

MEDICAL CONDITIONS AND INJURIES

The parent, guardian, or member (where applicable) must disclose any existing medical condition, illness, injury, allergy, or physical limitation that may affect participation in football activities, including but not limited to asthma, epilepsy, cardiac conditions, or previous injuries.

Where a medical condition or injury exists, the Club reserves the right to require written confirmation from a qualified medical practitioner stating that the player is fit to participate in competitive football activities without undue risk to their health or safety.

In the event of an emergency, the parent/guardian authorises the Club to obtain emergency medical treatment for the player where the parent/guardian cannot immediately be contacted. Any related medical costs shall remain the responsibility of the parent/guardian/member.
