Please note:  Details of the "Primary Contact Person" provide in the application who will be responsible for this registration, will be used to create an "user login" account.  In addition, the "Primary Contact Person" is also the person responsible for all legal terms and conditions between the parties.

For junior players (Boys and Girls Under 7 to Under 18), the Applicant must be the Parent or Legal Guardian of the player. For senior members, the Applicant will be the player or member registering on their own behalf.

These details will be used to create and manage the user account required to access the Club’s registration system, communication platforms, and related services.

Important Information:

• The Applicant is responsible for maintaining accurate and up-to-date contact information at all times.
• All official Club communication, including notices, fixture updates, and administrative correspondence, will be sent to the registered contact details provided here.
• For junior registrations, the Applicant will be considered the primary point of contact for all administrative and emergency communication purposes.
• For senior registrations, the Applicant will be responsible for their own account management and communications.

  • Transfer – Players currently or previously registered with another club who are seeking to transfer to MAFC.

This section is for the personal details of the MAFC member  or player. 

Please note, names and surname must be completed and spelled exactly as it appear on the ID documents for verification purposes.  Any variation may be rejected when the application is reviewed.

Once the Annual Subscription fee has been paid in full with proof of payment and all documentation & photos attached to application as tabled herein, your application will be accepted & submitted to SAFA.

NOTE: A clearance certificate is required if the player last played for another club prior to seeking registration with Meadowridge AFC

Please indicate previous club registrations details.   Please note that any player who were previously registered at another club (other than Meadowridge AFC) will require the following in order to be accepted and approved as a member of MAFC

• • Clearance Issued by the club on a letterhead.
  • Clearance issued by the LFA (could be the same as above)
  • The valid player registration card, or an indication (on the clearance letter) that the card has been lost.

Please provide the details of the primary and secondary contacts that the club may use to communicate club- and player-related information from time to time, as required. This may include notices relating to training, matches, fixtures, events, tours, disciplinary matters, emergencies, or general club administration.

Please also provide the relevant medical information, together with the details of the person to be contacted in the event of an emergency involving the member/player. This information is important to ensure that the club can respond quickly and appropriately should medical treatment, urgent assistance, or immediate communication be required during training sessions, matches, tours, or other club activities.

Parents, guardians, and members remain responsible for ensuring that all information supplied is accurate, complete, and kept up to date throughout the season, including any changes to contact details, medical conditions, or emergency contact information.

All information provided will be treated as strictly confidential and used solely for player welfare, safety, emergency response, and legitimate club administrative purposes.

Accurate contact details are essential for both club administration and emergency purposes. Please ensure that all contact information provided is current, complete, and regularly monitored.

• • Senior Members/Players
    Please provide the member/player’s own contact details as the primary contact. A secondary contact must also be provided for emergency purposes and should preferably be a spouse, partner, close family member, or another responsible adult.
  • Mini and Junior Players
    The primary and secondary contact details should be those of the parent or legal guardian. These contacts will be used for all administrative communication, match and training information, and emergency situations.
  • Junior Players Aged 16 and Older
    The player’s own contact details may be provided as the primary contact, subject to the approval of the parent or guardian. In such cases, the parent or guardian’s details must still be provided as the secondary contact and designated emergency contact person.

The club reserves the right to communicate directly with the primary and/or secondary contacts where necessary for operational, welfare, disciplinary, medical, or emergency-related matters.

Please provide full details of any medical conditions, allergies, injuries, or health concerns that the club, coaches, or team management should reasonably be aware of in the interest of the player’s safety and wellbeing.

Where applicable, please also provide:

• • Details of medication carried by the player (e.g. asthma pump, EpiPen, insulin, etc.)
  • Instructions in the event of a medical emergency
  • Medical aid/provider details and membership number

The section below contains details of the annual membership and subscription fees payable, including the relevant banking details.

The section also includes details relating to branded kit sales and sponsorship contributions.

Payment Plan
Applicants requiring subscription fee assistance must submit a written application to the Treasurer immediately on application for registration. This arrangement is intended to ensure that subscription fees are paid in full by 15 February for the forthcoming season, allowing members a total of six months to settle their fees in full.

This will allow sufficient time for the Club to submit and receive all registration cards from SAFA prior to the commencement of the forthcoming season.

No application will be accepted or processed unless the subscription fees have been paid in full.

I further undertake to ensure that my subscription fees are paid in full should I apply for a clearance during the current season or subsequent seasons.
